The F.D.A. found failures to disinfect equipment and prevent contamination after inspecting a Baltimore plant where J.&J. doses were ruined.

The Food and Drug Administration issued a highly critical report about the massive plant, operated by Emergent BioSolutions, which was forced to throw out millions of doses of Johnson & Johnson's coronavirus vaccine.

In a 12-page report, the inspectors cited a total of nine violations, ranging from the design of the building to improperly trained employees.
